| Details: Essential DutiesProvides therapy, case management, clinical and medical social work services to patients at CHCB's Safe Harbor Clinic as part of a medical team. Conducts rapid assessment, diagnosis, provides brief counseling support and longer-term psychotherapy, medical case management, crisis interventions and behavioral medicine interventions. Conducts social work consultations in order to assess mental health and substance abuse disorders and makes treatment recommendations. Assists with accessing benefits and resources including health and disability insurance, WIC, transportation and other resources as needed.  This is a 34 hour per week position.The Community Health Center of Burlington (CHCB) is a well-established, innovative Federally Qualified Health Center located in Burlington, Vermont.
